NHCP Offers Back to School and Sports Physicals

2 Aug 2012 | NHCP Public Affairs Marine Corps Base Camp Pendleton

 Naval Hospital Camp Pendleton will be holding special Back-to-School and Sports physicals for patients ages 5 to 19.

The first clinic will be July 10 from 4 p.m. to 7 p.m. for all eligible TRICARE Prime beneficiaries enrolled to the hospital and will be held in the Team 6 hallway of the Family Medicine Department. Patients can walk-in or call for an appointment at (760) 725-4357 and request the Sports and School Physical Exam clinic.

The next two clinics, July 21 and Aug. 4 from 8:30 a.m. to 2:30 p.m., are by appointment only for beneficiaries enrolled to the NHCP Pediatric Medical Home Port Teams. Patients must call (760) 725-4357, option #2 then option #2 again.

For all three clinics, patients must bring their immunizations record and the required physical exam form from their school or sports program.

Due to the focus of these clinics, medical care other than physical exams will not be available.
