ENROLLING IN DEERS
OR HAVE AN IDENTIFICATION CARD ISSUED

Q.  What is DEERS?

A. DEERS is the Defense Enrollment Eligibility Reporting System; registration is a crucial process for members of the U.S. Military and their eligible family members. DEERS is the primary system used by the Department of Defense (DoD) to verify and maintain the eligibility of individuals for military benefits, including healthcare and other entitlements. All documents presented must be unexpired, the original, or a “certified” document from the originator of the document.

 

Q.  What identity documents do I need to bring with me to get an ID card?
A.
 MCO 5512.11 F requires that you bring two (2) forms of ID; (1) being a valid unexpired Photo ID, i.e., Military ID Card CAC, Military dependent’s ID Card USID, Driver’s License, State ID, US Passport, or valid unexpired Foreign Passport. The second form of ID may be an original or certified birth certificate, original social security card (first time enrollment may use a printout from Social Security Administration office with the SSN on there with a signature from the SSA Rep and date stamp, no ID will be issued until actual SS Card is brought into the DEERS Office.), or a voter’s registration card.  If the names do not match on the documentation, an original marriage certificate, final judgement of divorce decree or an original or certified court order of name change must be brought in as well.

 

Q.  What would I need to renew my dependent’s ID card?
A. 
1. If you are already in DEERS and the sponsor accompanies you:

            A. Your old ID card (unexpired) and another form of ID

     2. If the sponsor does not accompany you:

            A.  DD Form 1172 prepared at a DEERS/RAPIDS site or present a DD Form 1172 signed by the sponsor whose signature has been notarized or a DD 1172-2 prepared by the ID Card Office Online with a digital signature, or present a current Power of Attorney (General or Special (note: the Special Power of Attorney must indicate authorization to enroll lawful dependents in military benefits programs and issuance of ID cards)

B.  Your old ID card and another form of ID

C. If the names do not match on the documentation, an original marriage certificate, final judgement of divorce decree or an original or certified court order of name change must be brought in as well.

 

Q.  What would I need to enroll my spouse for the first time? 
A. 
1. If you are already in DEERS and the sponsor accompanies you:

A. Your old ID card (unexpired) and another form of ID

     2. If the sponsor does not accompany you:

            A.  DD Form 1172 prepared at a DEERS/RAPIDS site or present a DD Form 1172 signed by the sponsor whose signature has been notarized or a DD 1172-2 prepared by the ID Card Office Online with a digital signature, or present a current Power of Attorney (General or Special (note: the Special Power of Attorney must indicate authorization to enroll lawful dependents in military benefits programs and issuance of ID cards)

Q.  What would I need to enroll a child or a stepchild in DEERS?
A.  
1. The sponsor needs to accompany or present a current Power of Attorney

     2. Certificate of Live Birth (within the first 30 days of birth) or Original Birth Certificate

     3. SSN card (first time enrollment may use a printout from Social Security Administration office with the SSN on there with a signature from the SSA Rep and date stamp, no ID will be issued until actual SS Card is brought into the DEERS Office)

    4. Marriage certificate. 

    5. Children under the age of 10 do not require a photo ID

 

Q.  What would I need to enroll a child born out of wedlock?
A. 
1. Sponsor must be present (No Cross servicing from other Branches of Service if you are not a Marine)

    2. Birth Certificate, or Certificate of Live Birth (Within the first 30 days of birth, then the Original Birth Certificate with the name of sponsor on Birth Certificate)

    3. SSN card (first time enrollment within the first 90 days may use a printout from Social Security Administration office with the SSN on there with a signature from the SSA Rep and date stamp, no ID will be issued until actual SS Card is brought into the DEERS Office)

    4. Court order establishing paternity or court order of support if it stipulates that the sponsor is the father of the child

    5. A voluntary acknowledgement of paternity filed through the state of where the child was born.
